Agent failures often pass every health check. A tool call hits the wrong database region, returns 200, and the user gets a slower, worse answer. Nothing alerts.
DigitalOcean’s new tools flag agent sessions that fail silently, then replay them against a fix before release.
Because, the clue behind an agent bug is often sitting in something nobody can search.
Signals watches every live session with cheap rules, not an AI model, that look for human trouble signs such as a user repeating themselves, asking for a human, or a tool being retried in a loop, then passes those sessions to Simulations, which replays them with a fake user against your fixed agent to check the whole conversation works before the fix ships.
Until now teams had to pay an LLM to grade every conversation, read transcripts by hand, or invent test cases from imagination, while here the real failures from production automatically become the test set.
DigitalOcean (@digitalocean): Catch it at the first user, not the four-hundredth.
A new user’s bank link fails, the agent says “try again later,” and nothing shows up in the logs. Watch a team find the bug and fix it before anyone else hits it, with Agent Learning Services and Agent Data Services, launching

"Your agent's failures are your regression suite" is great advice. Building eval datasets by hand isn't. Here's what I do instead.

Agents don't crash. They fail with HTTP 200, green health checks, and a polite "task completed"
AI agents can fail silently without traditional errors, as illustrated by a public postmortem where a pipeline ran into loops and high costs without triggering alarms. The article suggests using tracing and per-agent spend monitoring to detect such issues.
@rohanpaul_ai: Everyone shipped an always-on agent this month. And agents retry by reflex. Timeout, retry. Truncated response, retry. …
Orthogonal launched a service that helps always-on AI agents avoid wasting paid tool calls by checking usage after ambiguous timeouts or truncated responses before retrying, letting agents autonomously find, use, and pay for data services from many companies. The launch was highlighted alongside OpenAI's Dots integration for autonomous lead-generation pipelines.
The agent failures that get you aren't crashes. They're clean runs that did the wrong thing.
The article discusses how AI agents often fail silently by completing tasks incorrectly without crashing, leading to undetected errors. It highlights common failure modes and explores potential detection strategies.
