Dangerous failure mode in AI agents

A discussion of a dangerous silent failure mode in AI agents, where automations succeed technically but produce incorrect outputs that pass formatting checks, and what safeguards practitioners use to catch them.

I’ve been thinking about a failure mode in AI agents that seems more dangerous than a visible error The automation completes successfully, but the output is wrong. A few examples: An accounting agent assigns a transaction to the wrong category but returns a valid JSON response. A legal document agent extracts a clause incorrectly but passes every formatting check. A manufacturing workflow updates an inventory or maintenance record with stale information. An AI knowledge-base workflow retrieves a plausible answer from the wrong document. In each case, the workflow may show a green checkmark. The problem only appears later, when someone notices a bad report, a missed deadline, a client complaint, or an incorrect operational decision. I’m curious what people are using to catch this today. From our side, we've added an additional monitoring layer that checks the output against pre-defined business rules. What’s the most useful safeguard you’ve implemented for this?
